Who We Are:
Community Services Coordination Network (CSCN) coordinates access to services and supports for adults with a developmental disability, and for children and adolescents with complex needs in the Southwest region of Ontario. CSCN's dedicated staff connect, collaborate, and engage with individuals, families, service providers, and community partners to support people in navigating the complex landscape of developmental services and children's mental health support.
Who You Are:
We are looking for a true collaborator who is knowledgeable about accessing cross-sectoral community resources and has a strong understanding of developmental services and a commitment to person-directed and person-centred planning. You’re a proven facilitator, an excellent communicator, and you understand the importance of working with a diverse group of people with unique perspectives and experiences.
What You’ll Do:
The Service Navigator helps individuals with developmental disabilities and their families/caregivers to navigate systems, information, and developmental services funded by the Ministry of Children, Community and Social Services. Service Navigation is available at every step of the service pathway, from intake to application to accessing MCCSS-funded services. Additionally, the Service Navigator assists when needs or circumstances change while individuals are waiting for Ministry-funded services.
What Experience You Bring:
Ideally, you have a minimum of 5 years of experience working in developmental services directly supporting individuals, experience in service coordination, service navigation and/or case management, and post-secondary education along with the following:
•Proven exceptional knowledge of and ability to access community resources
•Demonstrated ability to work collaboratively with individuals, families, and multidisciplinary teams
•Strong emotional intelligence with the ability to handle difficult conversations and facilitate solution-focusedresolutions
•Demonstrated experience in working within a framework grounded in health equity, recognizing, understanding,and supporting the unique experiences of individuals and the unique barriers to accessing and receivingservices that members of racialized, 2S+LGBTQ, Newcomer, and other marginalized communities face andexperience in supporting them in a culturally safe and competent manner
•Excellent interpersonal and written communication skills
•Ability to work in an environment of change
•Previous experience presenting information to groups
•Proven ability to work both independently and collaboratively
•Ability to interpret legislation, government policy and guidelines
•A valid Ontario Driver’s License, access to a reliable vehicle, and the willingness to travel and work flexible hours as required
