Service Planning Coordinator- Coordinated Service Planning - CSCN Children's Services

London/Middlesex, Elgin/Oxford (2 Contract Positions - ending November 2026 and May 2027)

Who We Are:
Our key function is coordinating access to services and supports for adults with a developmental disability, and for children and adolescents with complex needs. Our role is to link individuals and families to community services and supports by providing information and exploring opportunities to enable full and fulfilled lives.

Who You Are:
We are looking for a true collaborator who is knowledgeable about accessing cross-sectoral community resources and has a strong understanding of the service coordination model and a commitment to person-directed, person-centred planning. You’re a proven facilitator, an excellent communicator, and you understand the importance of working with a diverse group of people with unique perspectives and experiences.

What You’ll Do:
The Coordinated Service Planning model is intended to improve the service experience and outcomes for children and youth with multiple complex special needs and their families. These young people may be experiencing challenges related to multiple areas, including physical, communication, intellectual, emotional, social, and/or behavioural development. The Service Planning Coordinator connects young people and their families to required services and supports, which may include services delivered in other sectors, various community-based services, and the services of multidisciplinary professionals. The Coordinator is responsible for developing, implementing, and monitoring a single Coordinated Service Plan with each young person and their family, in response to the individual’s goals, strengths, and needs. This includes coordinating and facilitating cross-sectoral team meetings as needed to support development, consistent implementation, and ongoing review of the Coordinated Service Plan.

What Experience You Bring:

The ideal candidate will have a minimum of 5 years’ experience working in children’s developmental and mental health services, post-secondary education in social services, and the following:
• Demonstrated ability to work collaboratively with individuals, families, and multidisciplinary teams
• Proven ability to develop, implement, and monitor coordinated service plans
• Excellent interpersonal and written communication skills
• Experience and demonstrated ability to facilitate meetings (virtual and in-person) with multiple interest holders with competing perspectives
• Strong emotional intelligence with the ability to handle difficult conversations and facilitate solution-focused resolutions
• Demonstrated understanding of issues impacting families of children and youth with multiple and/or complex special needs and the exceptional provision of high-quality, family-centered service coordination within the context of recognizing the diversity of individual families.
• Demonstrated experience in working within a framework grounded in health equity, recognizing, understanding, and supporting the unique experiences of individuals and the unique barriers to accessing and receiving services that members of racialized, 2S+LGBTQ, Newcomer, and other marginalized communities face and experience in supporting them in a culturally safe and competent manner
• Ability to work in an environment of change
• Proven ability to work both independently and collaboratively
• A valid Ontario Driver’s License, access to a reliable vehicle, and the willingness to travel and work flexible hours as required

We Offer:

You will work in a human-centered workplace built upon a culture that supports staff growth and learning. We promote work/life balance, belonging, and employee wellness and offer hybrid and flexible work arrangements. Salary Range: $55,851-$63,189.
