Our key function is coordinating access to services and supports for adults with a developmental disability, and for children and adolescents with complex needs. Our role is to link individuals and families to community services and supports by providing information and exploring opportunities to enable full and fulfilled lives.
Who You Are:
We are looking for a true collaborator who is knowledgeable about accessing cross-sectoral community resources and has a strong understanding of the service coordination model and a commitment to person-directed, person-centred planning. You’re a proven facilitator, an excellent communicator, and you understand the importance of working with a diverse group of people with unique perspectives and experiences.
What You’ll Do:
The Coordinated Service Planning model is intended to improve the service experience and outcomes for children and youth with multiple complex special needs and their families. These young people may be experiencing challenges related to multiple areas, including physical, communication, intellectual, emotional, social, and/or behavioural development. The Service Planning Coordinator connects young people and their families to the required services and supports, which may include services delivered in other sectors, various community-based services, and the services of multidisciplinary professionals. The Coordinator is responsible for developing, implementing, and monitoring a single Coordinated Service Plan with each young person and their family, in response to the individual’s goals, strengths, and needs.
What Experience You Bring:
The ideal candidate will have a minimum of 5 years’ experience working in children’s developmental and children's mental health services, post-secondary education in social services, along with the following:
• Demonstrated ability to work collaboratively with individuals, families, and multidisciplinary teams
• Proven ability to develop, implement, and monitor coordinated service plans
• Excellent interpersonal and written communication skills
• Ability to work in an environment of change
• Proven ability to work both independently and collaboratively
• A valid Ontario Driver’s License, access to a reliable vehicle, and the willingness to travel and work flexible hours as required.
